What To Bring To Your Appointment
At the time of your appointment, you will need the following:
- Your insurance card(s)
- Your driver’s license or picture ID
- New patient information and release forms
- List of current medications
Hygiene
Personal hygiene is especially important to help prevent infection. Please bathe before your appointment. If you have poor hygiene, you may need to be rescheduled.
Missed Appointments / No-Show Policy
Your appointment time is important to you, your physician, and others who need our services. If you cannot keep your appointment for any reason, please contact our office 24 hours prior to your appointment.
A late cancellation is considered when a patient fails to cancel their scheduled appointment with a 24-hour advanced notice. A no-show is when a patient misses an appointment with no notice or shows up too late to the appointment to be seen.
A $50 fee will be billed to your account for late cancellations and no-shows for all regular office appointments. A $100 fee will be billed to your account for late cancellations and no-shows for EMG testing, PRP, cosmetic Botox, and ultrasound-guided injection appointments.
You will be personally responsible for this charge. This charge will not be billed to or paid for by your insurance company. Future appointments will not be scheduled until this fee is paid.
You will receive an appointment reminder card with the date and time for your next visit. You will also receive a call to verify your appointment several days prior to the appointment date.
